For example, patients discharged from a Hospital at Home Program, may still have chronic conditions needing management after the acute period through a lower touch program, and vice versa. Or, an oncology patient may have fluctuations in their condition that may warrant different levels of care over the course of their treatment. How do we enable a seamless transition across their journey, and your enterprise while enabling care in the home?
